According to the question asked above,

- There are healthcare organizations in different countries who do not follow or are yet to follow the " do not use" list,hence the healthcare workers of these institutions when they move into an organization which is strictly following the " do not use" list, they are prone to make mistakes initially. Also lack of knowledge about new policies among healthcare professionals

- Possible.consequence being a medication error which could be dangerously affecting the patient's health

-There are many examples, one of which is using IU( international unit) which is mistaken for IV

- Daily inservice education

Make a list of "do not use" abbreviations, "look alike sound alike" list and display in every unit

Ensure 10 rights of medication administration

Follow proper medication reconciliation procedure

Always place a zero in front of the decimal point

Double check or triple check everything

Always give Intradermal test dose before administering medicines like antibiotics to check for allergy

Always be alert
