The main responsibility lies with the health care provider , not the patient to report cases of these diseases . CDC receives case notifications case notifications from 57 reporting jurisdictions . Each state has laws requiring certain diseases be reported at the state level, but is voluntary to provide information to CDC at the federal level.

The three main categories that cause diseases in humans are protozoa , helminths , and ectoparasites , and different types of transmission precautions , contact used for infections , MRSA , VRE , diarrhoeal illnesses , open wounds , RSV.

There are certain laws that impose penalties , in the form of either fines or jail terms , both who fail to report cases of suspected or neglect as required by the reporting laws .
