* Data can be used to evaluate outcome by

                       * Comparing the initial data of an individual or incident with

                          current information or data help to evaluate the outcome

                          of a program.

                       * Assessment of objectives and final results from the data

                          help to identify the effectiveness of a program or intervention.

Example;

           * A patient admitted in the hospital with severe abdominal pain and fever

             as a part of nursing assessment nurse assess the pain level and temperature

             daily using pain scale and thermometer and record it on the patient record.

             After five days of treatment nurse can evaluate the health care outcomes of

             patient by comparing the present pain score and temperature reading with previous

             days pain score and temperature.
