Nursing assessment : nursing assessment means gathering the client information like psychological, physiological, socialogical, and spiritual  through subjective and objective data, it is the first step in nursing process.

* it is used to prepare nursing interventions to meet the clients needs.

* after implementation of nursing interventions the registered nurse should evaluate the needs of client like whether meat the needs or not.

* if client meats his or her needs and outcome or evaluation is good, there is no reassessment of client information.

* if not meet the client needs there is a need to reassess the client from starting step of the nursing process that is assessment, to again up to evaluation.

* again she need to plan interventions and implement.

* because to meat comprehensive or complete needs of an client.
