State whether or not the following should be of concern to a laboratory supervisor and why: An Apt test performed on a black, tarry stool is negative for fetal hemoglobin.

Apt test is also known as alkali denaturation test. The test is performed on fetal stool or vomit to differentiate fetal blood from maternal blood.

A positive Apt test indicates that the blood is from the fetus. It could be due to GIT bleed, pulmonary bleed, vasa previa.

A negative test indicates that the blood is from the mother. The reason could swallowing of blood during breast feed or aspiration of maternal blood during delivery.

If the test is negative for presence of fetal haemoglobin than it means that the fetus is healthy and there is no bleed occuring in the fetus. But the blood is coming from maternal origin which could be suggestive of postpartum hemorrhage or breast fissure.

So the lab supervisor should not be worried about the fetus but he needs to be worried about maternal health.
