A client returned from theatre 20 minutes ago post bowel surgery with a GCS of 15. On examination of this client you find that their GCS has dropped to 10.

1) On examining the client with GCS dropped to 10 shows that the client conscious level is decreased, there will be chance to go for coma, So here the Nurse should Immediately check the ventilation and the airway of the client, the nurse may assist for normal breathing process.

The nurse also check the post medication and its dosage and its contra indication. Nurse should also asses the incision area for bleeding and if the condition gets worsen nurse should immediately approach the physician.

2) The possible cause for this condition are as follows,

• Post anesthetic effects
• some drugs interaction or contradiction
• internal metabolic disturbance leads to electrolyte changes
• some kind of fear or anxiety

3) The four members in the Emergency Response team in health care settings are

• Trained Physician ( Various dept )
• Emergency trained nurse
• Safety officer and their groups
• Radiology Technicians
