In reviewing his health history of smoking habits, all of the following indices should be affected. Hct, HgB, MCV, MCH, MCHC all could be elevated due to hypoxia as a result of smoking.

Based on the above information, his current folate and B12 status show he is in folate depletion.

Considerations:

  1. Serum folate level is under 18 nmol/L.
  2. The negative balance of red cell folate due to dietary changes.
  3. The red blood cells are macrocytic.
