Duals demonstration pilots are the scheme which cover both medicaid and medicare services. Some of the components of this project are providing timely care after hospitalization, effective communication, effective health education to the patients and medication management .These are the basic components they employed in the project. By this can reduce the hospitalization of the patients. Care coordination is seems high when compared to other schemes. Other schemes lacks the coverage of any of the services. So this project proves that it increase the care coordination in US.
