Patient is well known by the staff as being a “hard stick” and has been known to perform skin-popping when injecting heroin. Patient has a history of patellar tendonitis when taking quinolones in the past and previously developed anaphylaxis when given penicillin. What would be the optimal antibiotic regimen for the treatment of the cellulitis given his allergies?
A combination of a penicillin (amoxicillin), and cephalexin (cephalosporin) is the common treatment practiced for cellulitis. Patients sensitive to penicillin are also likely sensitive to cephalosporins. Cellulitis in these patients can be treated with clindamycin (lincosamide), and azithromycin or clarithromycin (macrolide antibiotics) that act by inhibiting bacterial protein synthesis.
