The plates which a nursing assistant can do before sending client for operation and then report to the nurse are as follow:

1. Take a thorough history collection and physical examination and maintain the chart

2. Surgical consent should not be taken by the assistant unless in the presence of an authorized nurse or physician

3. Consent for administration of blood should be taken by the nurse herself by explaining the procedure to the patient and the family members

4. Pregnancy test obtained when indicated can be done by the assistant and later report the procedure as well as the result to the nurse

5. Urinanalysis chart can be charted or maintained as per the report obtained from the lab and can be informed to the nurse

The procedures from chart preparation which can be done by the nurse assistant and later notify or inform to the nurse are :

- blood work which includes identification of the blood type and the cross matching can be done by the assistant nurse

- known allergies and sensitivities can be noted on the chart which is related to the patient obtained by collecting the related history from the patient

- known exposure and or allergies to the latex can be noted by the assistant in the chart and to the nurse

In the patient preparation the procedure which can be done by the nurse assistant and notify later are as follow:

- family notification about the surgery which can be done by letting the family write their names , date and their signature where the notification can be done by the nurse and the physician and the notified signature or consent can be taken by the assistant

- patient's identification band on wrist can be put by the assistant with the help of the patient chart

- removal of the prosthesis including dentures, wigs, hairpins, contact lenses, cosmetics, nail polish, artificial eyes, limbs etc can be removed by the assistant by informing the patient and the family members

- removal and handing of all the jewelries should be done by the nurse with the signature of a standby and a family member

- clothing can be removed by the assistant and don the hospital gown by the assistant

- time of last fluids or meal can be charted or reported by the assistant

- pre op TPR and BP can be checked by the assistant and later notified and recorded in the chart

- voiding time of the urine by the patient can be informed and recorded in the chart
