The pre-surgical dose is 30 mg of anticoagulant drug

Patient weight is 140 lbs, to convert lbs to kgs divide the mass with 2.2

                          = 140 \div 2.2

                          = 63.6 kgs

To get the dose per kg body weight divide the dose with total body weight

                            = 30 \div 63.6

                            = 0.47 mg

The patient was given 0.47 mg per kg body weight of anticoagulant drug pre-operatively.

The post-surgical dose is 1 mg per kg of body weight

                         = 63.6 kgs \times 1 mg

                         = 63.6 mg

The patient should be given 63.6 mg of anticoagulant drug post-operatively.
