Foley catheter:
Indication:
Foley catheter used to drain bladder in case of urinary incontinence, retention, post surgery, urethral obstruction during any abdomen surgery, any investigation and to promote perineal healing by avoiding skin breakdown.
Nursing intervention(pre, intra, post)
-confirm patient identification
-Make privacy for the entire procedure
-Explain the procedure to the patient and indication
-Keep the equipment ready and catheter as per the size and age of the patient.
-Follow aseptic technique and check the catheter blockage and balloon inflation
-Lubricate the catheter tip before insertion
-Listen to patient words during the procedure
-check patient vital signs
-secure catheter in the patient leg
-dispose of the supplies after the procedure
-wash the hands after the procedure
-document in the patient record
evaluation:
check urine output, patient tolerance of the procedure and any unexpected outcome like bleeding, pain or any obstruction, redness.
Client education:
- Advice patients never try to pull or push the catheter to avoid urethral injury.
- Inform if nay sudden rise of temperature, bladder pain, nausea, vomiting, bleeding in the bag, catheter comes out, no urine for last 6 hours for any unusual symptoms
-Advice the patient to shower daily if possible and avoid scented soaps.
-Be sure the tube never kinks
-empty the bag every 2 to 4 hours. follow hand hygiene.
Potential complications:
Hematuria, fever, chills, headache, low back pain, burning of the urethra, pus in the urine.
Nursing intervention:
Educate the patient about catheter management
Monitor urine output and watch for any complication.
