# Brief statement of need for query - In this case, query is raised because post surgery the patients hemoglobin and hematocrit has been dropped than pre surgery blood levels . To compensate this the physician has ordered for transfusion of 3units of packed red blood cells and in the discharge summary there is no mention of the blood transfusions which were given . For this reason the query is raised because mentioning the blood transfusions is very important as it is an intervention and also if any reaction occurs management will be made according to that .
