She states that her pain is most intense during the morning, upon arising from bed but tends to improve with movement. She states she “warms up” during the day, but if she overexerts himself, her morning stiffness is worse. On physical exam, you note tenderness at the right hip and knee with movement but no swelling or erythema. You examine her right knee and note crepitus with flexion and extension. You order an X-ray:

1, Most common cause for this pain due to joint injury or obesity,genetics,race and gender..Joint can bear the weight..But when it extend for few months it can affect the joint cartilage it can gradually breakdown when you are overweight..It will be painful and we can grating sound when bones rubs together..
2, Obesity and it is mostly common among African-American women..Osteoarthritis is a common joint disease it affect hip joint,knee joint, hand and spine..it is associated with aging and obesity..African American more likely to have knee symptoms due to obesity and depression and racial difference..
3, Osteoarthritis commonly affect the joints like knee,hip,hand,foot and ankles,thumbs,big toe,back their toe..
4, Crepitus caused by damage to the knee joint..pain as the knee snaps it can be scan tissues,a meniscus tear or tendon moving on the protruding bone with in the knee joints..this issue need medical attention immediately..
5, Basic treatment include lifestyle modification like exercise,diet control can reduce the weight and avoiding alcohol and smoking..
Medication like analgesics for pain and non steroidal anti inflammatory drugs(NSAID) like Aspirin,ibuprofen,naproxen and corticosteroids..
Physical therapy like range of motion flexibility exercise,heat and cold therapies and using assistive devices like walkers,splints can help with function and mobility..natural remedies like nutritional supplements,massage,relaxation therapy and surgery for repair hip and knee if joint severely get damaged..
