A CARE PLAN provides direction and serves as a guide for the healthcare team in order to best help the patient. Most care plans only address the patient's medical needs. make a list of 10 QUESTIONS you think are important for your healthcare team to know that are not on the standard care plan. A care plan provides direction and serves as a guide for the healthcare team in order to help the patient in better way. Most care plans address the patient medical needs. So I will be adding some points other than medical needs so that it will help healthcare team to provide holistic care to the patients.

  1. First and for most will be healthcare team should know demographic data of the patient like name, sex, age and religion so that it will be easy for the care giver to address the patient.
  2. She or he should know about patient condition and diagnosis so that she will provide appropriate care as per the needs.Also she should have knowledge about that condition.
  3. Before starting daily care of the patient, greet the patient poiltely so that care giver can gain confidence or trust of the pateint.
  4. Make an agenda for the day like what care will be given for the day.
  5. Primarily start with assessing patient vital signs and document it.Then continue with head to foot assessment systematically so that healthcare provider will get an idea if any futher care needed as per system wise.
  6. Collect any laboratory reports or radiological reports which have been done on that day.If any critical reports then inform concerned person or phyisican and carry out orders as per thirer advice.
  7. Monitor critically ill patients periodically. Any bed ridden patient give proper nursing care incluiding ancillary care.
  8. Provide a better diet plan as per dietician orders according to the time given.
  9. Get help from physiotherapist and do some physio's to patient who needs the most.
  10. Get help from pharmacologist for the better guidence in medications.
  11. Do rounds with consultant and give care as adviced.
  12. If the patient is ok for spritual belief then get some consultation for spritual leaders
  13. If the patient is very sick then provide counselling with the help of psychologist.
