Develop a comprehensive discharge plan, based on the criteria listed below for the client and family.

All plans must be supported by credible sources.

The plan must include:

  • Patient data (age, diagnosis, procedures, and caregivers);
  • Family structure and support
  • Family resources (housing, transportation, financial concerns/insurance)
  • Cultural and religious considerations
  • Medication reconciliation
  • Equipment
  • Education (physical needs, medications, and available resources)
  • Follow-up plans/care agencies
  • Health maintenance and disease prevention

Grammar and spelling -2 percent will be deducted from the final grade if there are three or greater grammar and/or spelling errors. .APA errors-2 percent will be deducted from the final grade if there are three or more APA citation and/or reference errors.

Family structure : nuclear gamily .

Family support : patient husband is bread winner of house .

Housing : pakka house

Transportation : they are using public transportation like bussess for travelling.

Financial concern : poor family under BpL .

Cultural and religious considerations : they believe jejus christ and every sunday they Use to go for church for prayer .

Medication consillation :

Tab : trenexa

Tab : misoprostol

Tab : be complex .

Procedure : pervaginal examination not done for patient because vaginal examination further provokes the vaginal bleeding .

Simple speculum examination done .

Patient education :

Mother is adviced to regular followups .

Mother is adviced to consult midwife when ever emergency happen .

Mother is advised to avoid heavy weight lifting .

Educated to take plenty of fluids to reduce dehydration educated to take iron rich diet and high nutritious diet .

Educated to take immunization if not done previously .

Educated to take IFA Tablets regularly .

Adviced to go for coagulation profile and screening for hemolytic diseases

Follow up plan :

Patient adviced to regular follow ups .

Educated to come for 22weeks once for regular check ups to know the pregnancy events .
