Directions: The student will utilize Marjory Gordon’s Functional Health Patterns as a template for self-assessment. By applying a theoretical model of change, the student will also identify a desired health-related goal for self and the process to achieve that goal. The student should not disclose any personal information that the student determines to be sensitive in nature and/or private or confidential. For example, the student should not disclose that he/she is taking a specific medication or consulting with health care professionals for his/her health care needs. Rather, the student may wish to record a more broad-based statement like, “I am going to increase my exercise to meet national nutritional guidelines.” The above directions will be discussed in class. If a student has any questions about completing the assignment, the student is encouraged to consult with the course Instructor. The student is also encouraged to submit a draft of the paper, for Instructor feedback, at least one week before the due date of the assignment. This paper is worth 100 points CONTENT TO INCLUDE IN THE PAPER: (If so desired, the student can provide hypothetical data about self for any or all of the data that are required in the paper) 1. Your: age; gender; height; weight; temperature; blood pressure; heart & respiratory rate; 2. General description of yourself. This should be a short paragraph. 3. Identify your developmental state according to the major theorists. You may use a chart for this. Format with column one Theorists; Column 2—your developmental stage according to the theorists; Column 3—any associated tasks for that period and where you are (This may suggest something you need to work on, i.e. an insight leading to a change item). 4. Coping strategies used and their effectiveness. Personal losses or major stressors in the last year. Comfort and security needs. Tobacco use. Alcohol use. Other substance abuse. VALUE-BELIEF Describes patterns of values, beliefs (including spiritual), or goals that guide choices or decisions Religious/cultural/spiritual practices. Sources of strength and hope. Life goals. Spiritual needs generally and during time of stress. Need for clergy /support persons. 5. Select the one thing you would most like to change. Summarize why you have chosen this item to change, improve, or work on in your life. Identify the steps you will take to make this change. Identify a time frame for the change. Discuss the things in your environment that will help you make this change. Discuss the things in your environment that may hinder your change and if you think you can do something about them. Don’t forget to identify a change theory (such as Bandura) and how you see what you are trying to do fitting into that theory. This summary should take a 1 to 1 ½ pages. To summarize, the paper should include: 1. An introduction “to you” including a general description of yourself 2. Summary Charts a. Developmental Assessment b. Functional Health Pattern Assessment 3. A summary of a proposed change “for you” (see # 5 in the preceding) Most papers end up being approximately 10 pages in length. Majory's Gordon's functional health pattern is developed by Majory Gordon.It is a method used by the nurses in the nursing process to provide a comprehensive nursing assessment of the patient. Gordon's functional health pattern includes 11 categories which is a systematic and standardized approach to the data collection such as :

2. General Description:

My self Mrs.X,23 years,presently suffered with lung GVHD. 2 years back for it is diagnosed as AML (Acute Myeloid Leukemia),and underwent Bone MArrow transplant and is presently suffered with chronic GVHD of lungs.
