The Edelman and Kudzma (2017) textbook introduced you to the Gordon’s Functional Health patterns: Assessment of the community model, but can you identify another community assessment model/framework that could also be used to evaluate the risks/health status/needs of a community? Please use evidence to support your discussion. Identify resources in your community that would enable you to complete a community assessment. Submit a summary of findings.

A community need assessment is an important process to identifies the strength and resources available in the community to meet the needs or demands of the community people.Through this assessment we can effectively use the available resource to meet the demands.Through the assessment process we can identify the health status,needs of the community in a systematic way.

CHIP Model

CHIP stands for Community Health Improvement Process

The CHIP is a community Health Assessment model helps to monitor the overall health matters and can address the specific health issues

The CHIP model or frame work consists of two primary interacting cycles.

The first cycle is a problem identification and prioritization cycle,through this process we can assess the situation,identify the areas of concern (area of issues) according to that we can put the effort to the second cycle

The second cycle is the analysis and implementation cycle,here we are purposefully utilizing the information gathered from the first cycle.Through this phase we can analyze the issues the community is facing and the factors that leads to the particular issue then based on that we can plan for stretegy,then implement it and atlat monitor the process outcome.Again if needed we can go with the first interacting cycle that is problem identification and prioritization cycle In the problem identification process there is a co-operation needed to identify the problem thus to prioritize it.

We have utilize all the available resources in our community to reach the effectiveness.For the better outcome the stake holders and the organiztions put an efforts to analyze the problem and and effective implementation of the planning.Implementation can be done at the organizational level
