Step 1: Development of Basic Skeleton Diagram

In this case assessing the patient's health history , demographic data social economic status , lab reports etc

Step 2: Analysis & Categorization of Data

In this after collection of data , analysing it and putting it different categories

Step 3: Labeling, Prioritizing, & Analyzing Nursing Diagnoses Relationships

According to the need of the patient prioritizing the needs and planning implementation needed .

Preparing the nursing diagnosis like - Anxiety related to the diagnosis

Step 4: Identification of goals, outcomes, & intervention

Preparing g objectives and goals for the patient care based on needs . Like reducing the anxiety related to diagnosis

Step 5: Evaluation of patient's responses

In this case ,after implementation of the care , evaluating the response , are the goals achieved or not etc
