While in treatment his labwork revealed an elevated A1C, with a subsequent diagnosis of Type II diabetes. Mr Abrams is a roofer and has a family business. Per the chart history he may consume 3-10 beers daily (“depending on work schedule”). He has a history of HTN x 20 years. Mr Abrams lives with his wife of 35 years. You received a consult for 60 gram carb controlled diet and diet education for this patient. His diet was changed from a regular to 60 g carb following diet progression/tolerance (now x 2 days). The diet aide has shared with you that he has been asking for Dr Pepper and complaining about not being able to receive on his tray. Ht: 5’9” Current Wt: 262 lbs Lab: Hgb A1C 8.7%; BG 232 mg/dl; T.Chol 220 mg/dl; Trig 266 mg/dl BP: 138/90 Meds: Zosyn, Lisinopril, pantoprazole

Assessment and dietary recall:

  • The diet recall help to analyze the patient's diet intake in the last 24 hours.
  • Ask the patient to list the diet intake previously including of any beverages.
  • A dietician should also ask about the preparation method (baked, boiled, fried) and the amount and frequency of intake.
  • Ask about the intake of any dietary supplements.
  • Assess patient dietary patterns and eating habits.
  • Assess the physical activity and carbohydrate burn out.
  • Assess the patient's lifestyle activities and nature of work.
  • Ask the patient to describe the appetite level.
  • Ask the patient whether he skips the meal or snack.
  • Ask the patient whether he left over any food or particular food item.
  • Ask the patient whether he feels tired or lack of energy and to specify the situation and time.
