1. Generic name - Levofloxacin

2. Brand name - (the Same drug is manufactured by many companies under many names) Levaquin, Levomac, Benlevo, Diolof, Gevoflox, Glevo, etc.

3. Dosage

Adult dose

  • Hospital-acquired pneumonia - Intravenously or Orally 750mg Once a day
  • Pneumonia - Intravenously or Orally 500mg Once a day
  • UTI - Intravenously or Orally 750mg OD
  • Skin infection - Intravenously or Orally 750mg- 500mg OD (According to severity)

Pediatric dose

  • Anthrax prophylaxis - 8 mg/kg Intravenously or Orally in 12 hrs (max dose of 250mg)
  • Plague prophylaxis - 8 mg/kg Intravenously or Orally in 12 hrs (max dose of 250mg)

There should adjustment of dose in CKD(Chronic kidney disease) according to creatinine clearance

4. Route - Intravenously or Orally

5. Frequency - 12 hourly or 24 hourly

6. Classification - Fluoroquinnnolone group of antibiotics

7. Indication - Bacterial Infection

8. Common adverse effect - Vomiting, nausea, diarrhea, abdominal cramp, insomnia, headache, etc.

9. Life-threatening adverse effect - clostridium difficile associated diarrhea, Kidney failure, Jaundice, and liver failure.

10. Dug interaction - Birth control pills(Decrease effectiveness of birth control pills)

Ofloxacin

11. Food interaction - No food interaction

12. Nursing interaction -

  • Levofloxacin and ofloxacin are very similar, do not use them together.
  • Maintain Right patient(Check patient identification whom you going to administer), Right drug(Levofloxacin), Right dose(According to disease condition), Right route (IV or orally), Right time.
  • Evaluate the therapeutic effect by seeing WBC count, signs of infection(fever), and other signs symptoms.
  • Give education regarding common side effects of levofloxacin and tell them if there are any critical changes in the body like changes in the amount of urine or yellowish discoloration of skin then see the doctor immediately.
