Sometimes technology can create barrier between the physicians and patients. In past years the physicians maintained the medical records on paper charts and stored in their office with closed contact with the patients on periodical basis, but due to implementation of the EMR (electronic medical record such practice of physician to patient interaction is reduced as EMR has created a barrier in this process.

Hence it is indicator of reduced patient to doctor contact due to technology/barriers and does not improve efficiency and productivity.

Thus Option A is correct.
