Nursing Diagnosis

Activity intolerance related to decreased blood sugar level as evidenced by dizziness, confusion, increased appetite,decreased blood sugar value,impaired judging

Pathophysiology

When the blood glucose in the blood is below the normal limit,the supply of glucose to the brain is decreased,for time being the brain gets glucose from astrocytes for few minutes.Ty patient gets the first symptom of dizziness,loss of balance,fatigue,perspiration .Later the stored glycogen from liver is released as glucose.If patient is not given glucose from external source then he brain will lack glucose to function leading to symptoms like confusion,disturbances in judgement, impaired activity ,seizure , irreversible loss of consciousness coma and death.This incident is termed to be neuroglycopenia.

Risk factors

  • Medication ( insulin)
  • Not eating food
  • Age
  • Long standing diabetes
  • Reactive hypoglycemia
  • Alcohol

Expected findings

  • CNS:confusion, impaired memory, poor judgement,anxiety,headache
  • CVS:palpation
  • GIT: nausea, vomiting, dry mouth, hunger
  • Musculoskeletal:tremor,loss of balance,perspiration,pale skin
  • Sensory:blurred vision

Laboratory tests:

  • Hypoglycemia <60mg/dl
  • Neuroglycopeneiq <10mg/do

Diagnostic procedures

  • Blood sugar testing while experiencing tye symptoms
  • Physical examination
  • History collection

Health promotion and disease prevention

  • Regular tracking
  • Knowledge about the drugs taken and its effects
  • Encouraging to avoid alcohol
  • Use wearable devices and alert bands
  • Eat meals on time ,never skip

Safety considerations

  • Keeping glucose in handy

Patient centered care

Nursing care

  • Monitor the blood sugar level
  • Administer oral glucose or zinc Dextrose as per order
  • Assess for complications
  • Record the blood sugar levels
  • Monitor GCS

Medication:

  • Intravenous Dextrose solution
  • Stopping any medication which can trigger insulin production

Therapeutic procedure

Administration of tye appropriate level of glucose

Client education

  • Health education about the signs and symptoms
  • Encouraging to eat
  • Carry extra carbohydrate foods

Inter professional care:

  • Nutritionist to plan the diet
  • Endocrinologist to rule out and hormone related problems
  • Diabetologist to plan the entire treatment regimen

Complications

  • Coma
  • Death
