Bays HE. Adiposopathy, diabetes mellitus, and primary prevention of atherosclerotic coronary artery disease: Treating "sick fat" through improving fat function with antidiabetes therapies. American Journal of Cardiology.2012;110:4B-12B.

This article examines the connection between diabetes, cardiovascular disease, and excessive body fat (adiposopathy). The author discusses the effectiveness of drug therapies for diabetes in terms of their effect on the “endocrine organ” of accumulated fat stores, and the effect of these fat stores on the development of cardiovascular disease (CVD). Consider the following questions: what evidence does the author give for the connection between diabetes, CVD, and “sick fat”? Which medications prescribed for diabetes are most effective in preventing CVD, and why? Why does the author dismiss the strategy of weight reduction for diabetic patients? What evidence does he show for his claim that what we eat (and how our gut reacts to what we eat) can influence the development of “sick fat” and CVD?

Answer #1

Answer 1

The author considers endocrine organ as the connection between diabetes cvd and sick fat as a defect in insulin(hormone) synthesis is directly related to development of diabetes which is followed by adipose tissue depisiyiod ultimately leading to cardio vascular diseases.

Answer 2-

Medications prescribed for diabetes that are most affecting in preventing cvd includes-

1. Anti hypertensives-

Beneficial effects io lowering BP has been established especially in those with compelling indications like diabetes as it had proven to reduce cvd by reducing the diastolic BP up to 90mm hg

2. Lipid altering drugs-

The hypoliidaeminc drugs have the potential to prevent cvd by retarding accelerated atherosclerosis

3. Anti platelet-

Anti platelets interfere with platelet productuon and diabetes is a condition with progressive arteriosclerosis, hence anti platelets Block one or multiple pathways that interfere with platelet production and aggregation reducing the risk of ischaemia

Answer 3-

The author dismisses the strategy of weight loss in diabetes through obesity paradoxes as in recent researches it has been found that weight loss did not have any effect on the occurance of stroke or any other cvd although it does improve the Hba1c levels and improves the quality of life

Answer4

What we eat can influence the development of sick fat and Cvd as clearly stated by the author that sick fat is influenced by positive caloric balance and sedebtase lifestyle in genetically and environmentally susceptible patients which in turn promotes a metabolic disease

It says how excessive calorie intake results the fat to go sick and recommends weight loss as an effective therapeutic modality to improve metabolic disease and obesity
