So basically to start with answering your question in ome important phases

Phase 1

It is known that the basic evolution happens with the process of natural selection as well as the modification an individual undergoes with the period of time. These modifications are being carried forward or being inherited from parent to the offspring.

PHASE 2

The physical form of environment in which an organism lives makes the individual to kind of adapt for the survival which ultimately results in the bodily modifications

The terms such as the genetic drift, development of new specie which is the Speciation and also the form of developing new things which can be referred to as the Innovation etc are all a result of this phase. It is also to be noted that not every organism can adapt to the changes in surroundings , there are some organisms that cannot withstand the survival as well.

This theory has already been explained by the DARWIN, which is the result of the variations to be more specific it is the Mutation. An individual which kind of made its adaptations for the survival has undergone one or more mutations and is referred as the survival of the fittest. Those which cannot make this survival will eventually die

PHASE 3

Now to illustrate an example

The so called antibiotics that are utilized in order to control or inhibit the bacterial growth and which eventually help the organism in curing from the infection, when used in high doses makes the bacteria to become resistant to the antibiotics.

Most of the bacteria have become resistant to the antibiotics. From this it has to be understood that antibiotics now cannot affect the living of bacteria in any form

Now moving onto the second part of the question

Natural selection play its utmost role in developing the organisms with that of the environment but there is also a part where there are some errors in the process of coping with the development that is the bodily changes in the organism. The reason for this is that , this process of development in the organism does not start from the beginning or from the basic changes . It is something that happen from already developed or evolved individual in accordance with the previous environment. All this happens on the basis of natural selection, hence there are some chances of imperfections as mentioned in the question .

Like for example in the case of Bats, Chiroptera the wings of the organism are not so good enough that is the structure of the wings are not so perfect enough for its flight . But there are enough of evidences that its forelimbs have undergone modification which they usually use for the purpose of walking. The natural selection is not only the sole reason for the imperfections it is the difference in the genetic makeup which is because of the multiple activities that organisms do, the perfect form of body or design is not possible for all the specific activity they perform
