12. The amount of the drug which is received by a child in a day is called as daily dose

13.It is incorrect because first the name of the medicine to be written followed by dose ,route and frequency

Tylenol 650mg PO, q4h PRN

14.Given

Order:Vancomycin

Dose:200mg

Route :IV

frequency :Four times a day

a. Weight :35lb(35*0.454=15.89kg)

Upon rounding off it is 16kg

Ans.16kg

b. Safe dose

Recommended dose:40mg/kg/day

As 1kg=40mg

Then 16kg=??mg

Cross multiply and divide

=16*40/1

=16*40

=640mg/day

Ans:The safe dose for the child in a day is 640mg

c. The divided doses of 640mg is 640/4=160 mg each dose

d. This is not the safe dose because the given order states 200mg/dose

So 4 dose:200*4=800mg/day

It has exceeded the safe dose.
