Answer :
Myelofibrosis is a condition of fibrosis of bonemarrow this will afftect production of all cell lineages.
The counts will vary according to stage of disease. There are pre fibrotic, overt stages. Here Iam mocking an overt stage CBC of myelofibrosis.
WBC - 3500cells/ul (Count mildly decresed)
RBC - 3lakh/ul (Count decresed)
Platelet - 0.9 lakh/ul ( mild thrombocytopenia)
Hb - 9 g/dl (decreased)
Hct - 30%
MCV - 82
MCHC - 30
RDW - 13
MPV - 20 ( Increased as there are giant platelets)
N - 56%
L - 25%
E - 09%
B - 1%
M - 3%
Myeloblast - 6%
Differential smear.
WBC - Count mildly decreased with predominant neutrophils. Myeloblasts seen. Left shift of neutrophils noted.
RBC - Normocytic Normochromic
Normoblasts seen.
Platelet - count decreased with giant forms and anisocytosis.
Cytogenetics - CALR mutation, inv(3), del(5q), JAK2 V617F mutation
Special stain - reticulin stain can be used to demonstrate myelofibrosis.
NB - In initial stages wbc count and platelet count may be increased.
