Transitional care is an intervention and a group of interventions initiated prior to the hospital discharge. Transitional period includes 3 phases

Phase 1-it is the first period of reactivity.it could be in delivery room.phase I starts from birth and lasts up to 30 minutes.

Baby is alert and acive vs unstable respiration

Phase 2-sleep inactive period,from 60 to 100mts.

Baby is sleepy and decreased activity.

Phase 3- second period of reactivity.baby may be with mom,2 to 8 hours after birth lasts 10 minutes to several hours. Baby is alert and acive again, period of tachycardia and tachypnoea and increased muscle tone,skin colour,mucus production and baby passes meconium

Task/assessment during transitional care periods-

-Maintain cardiac respiratory functions

- Maintain thermoregulation

-Assess apgar score

-Apply cord clamp

-ID the mother, father and newborn

-Perform brief assess for major anomalies

-Encourage bonding

-Encourage breast feeding.
