Background information and previous aural rehabilitation:
Two years ago, Mr. Wolfe contracted bacterial meningitis. He had been in good physical shape, but 10 days later, he woke up from a coma in a hospital, unable to hear conversational speech. “Obviously, it was a pretty traumatic experience, happening overnight basically,” he reports. He received counseling from a psychologist for about 6 months. He reported that the counseling was helpful, but that he believed that the psychologist hadn’t been very empathetic or insightful about his new communication difficulties. His hearing loss, depicted in the audiogram below, can be described as a symmetrical sloping mild-to-profound sensorineural loss. His SRTs are 65 dB HL in each ear, and his scores for the CID W-22 word lists in an auditory-only condition are 80% words correct in the right ear and 75% in the left ear. Within 3 weeks of being diagnosed with a hearing loss, he underwent a hearing aid fitting. He now wears two BTEs. His aided thresholds for the Iowa Sentence Test administered in an auditory-only condition in quiet is 86% words correct.
Mr. Wolfe is divorced. One of his teenage sons lives with his wife in a city about 500 miles from his home and the other lives with him.
Work situation:
Mr. Wolfe is a trial lawyer. He is considering applying for disability retirement because of his hearing loss. He is concerned with the consequences of doing so though, because outside of work, he has few interests. “What am I going to do with myself if I’m not working?” he asks. His son is a senior in high school and will soon be leaving to attend a university in a different city.
Hobbies, social activities, and other interactions possibly affected by hearing loss:
Mr. Wolfe used to enjoy attending BMW car shows. Now he says he doesn’t enjoy these kinds of events because they often entail talking to others in groups, which he finds difficult.
Reason for self-referral:
He is satisfied that he is fitted with an optimal hearing aid. However, he realizes that he has been feeling distressed about his hearing loss and about the prospect of losing his job because of his communication difficulties.
