Pediatric safe dose range is calculated through following steps:

1. Convert weight from lbs to kg (here already given in kg)

2. Multiply low range given in the problem (of recommended dosage) with child’s weight. Then multiple high range given in the problem (of recommended dosage) with child’s weight. It will give minimum and maximum value of safe dosage range.

3. In case of 24 hour recommended dosage, divided into single dose which is given every 12, 8, 6, or 4 hours, first calculate the 24 hour dosage, then divide it by the number of times dose is given per day.

Ampicillin

Child weight 4.2 kg

Minimum safe dose range = 100 X 4.2 = 420 mg/24h

Maximum safe range = 200 X 4.2 = 840 mg/24h

Safe dose range for 24 hour is 420-840 mg/24h

Single dose range is 420/4 = 105 mg/dose (minimum) and 840/4 = 210 mg/dose (maximum)

Single dose range is 105 – 210 mg/dose.

Vanomycin

Minimum safe dose range = 15 X 4.2 = 63 mg/dose

Maximum safe range = 20 X 4.2 = 84 mg/dose

Safe dose range is 63-84 mg/dose

Acetominophen

Minimum = 10 X 4.2 = 42 mg/dose

Maximum = 15 X 4.2 = 63 mg/dose

Safe dose range is 42-63 mg/dose

Ibuprofen

Minimum = 5 X 4.2 = 21 mg/dose

Maximum = 10 X 4.2 = 42 mg/dose

Safe dose range is 21- 42 mg/dose
