Chronic obstructive pulmonary disease:
Nursing Diagnosis:
-Ineffective airway clearance
-Imbalanced nutrition
-Impaired gas exchange
-Deficit knowledge
-activity intolerance
5 nursing interventions:
-Make the patient comfort with elevated headend in the bed to facilitate respiratory function.
-Teach abdominal breathing, coughing and deep breathing technique to reduce dyspnea and improve ventilation.
-Increase patient fluid intake within cardiac tolerance, provide warm liquid and humidified oxygen to loosen the secretion and reduce the bronchospasm
-assess the patient breathing sound and pattern to identify infection and bronchospasm.
-Improve knowledge of the patient to avoid allergens and follow the treatment regimen correctly.
5 outcomes:
-Maintain a clear airway pattern with clear breathing sound.
-Correct treatment followup
-Improve airway clearance by demonstrating the behavior.
-Improve oxygenation by effective use of techniques.
-Avoid further complications by baseline monitoring and use grade of breathlessness scale.
