The nurse is preparing to administer D5NS IV to a patient admitted with an Upper GI bleed. Use the concepts about the movement of body fluids and tonicity (osmolality) to explain how this treatment can benefit the patient.

Answer : upper GI bleeding is a potentially life threatening abdominal emergency that remains a common cause of hospitalization .

Upper GI bleeding is defined as bleeding derieved from a source proximal to the ligament of treitz .

If the patient is hemodynamically stable ,We can provide oral fluids .

If the patient is not stable ,We need to give iv line such as NS and D5 which ever required according patient response.

  • To treat low extravascular fluid
  • D5 is considered as an isotonic solution ,but when the dextrose is metabolized the solution becomes hypotonic and causes fluid to shift in to cells and it help to prevent from shock .
  • D5 provides free water that pass through membrane pores to both intracellular and extracellular spaces .its smaller size allows the molecules to pass more freely between compartments ,thus expanding both simultaneously.
  • D5 NS provide fast response to hypovolemic shock ,so We can Use in upper GI bleed.
