Q.1- A nurse is preparing to administer a litter of normal salaine to be infused from 07.00 to19.00.The drop factor is 20 gtts/ ml.The client went to a procedure and was disconnected from iv from 13.45-16.15. The client came back at 16.15 and was restarted to iv fluids so how will the nurse set the IV pump rate if the fluids needs to be completed at 19.00 ?
Ans- Here, plan for one liter of normal salaine to be infused from morning 7 am to evening 7 pm in drop factor 20gtts/ml.
Rate is=Diseired volume× drop factor / time in minute
So here Diseired volume is 1litter=1000ml
Drop factor is=20 gtts/ml
Time is 12 hour=12×60= 720 min
Rate is = 1000×20/720
=27.77 drops /min
=28 drops /min approximately
If 27.77 drops /min infusion is going on then it will complete till 7pm,But infusion is going on till 3.45 pm due to procedure.
So in 27.77 drops/min is continue till 7am to 3.45pm
That is 8 hour 45min=8×60+45=525 min
So here volume to be infused is
Volume =rate ×time/ drop factor
=27.77×525 min/20
=728.96ml
=729 ml aprroxmately
729 ml fluid already transfused till 3.45 pm
Then stop fluid till 4.15pm
So it's 30 min.
Then again fluid continue till 7pm
This time is 4.15pm to 7pm= 2 hour 45 min
= 165 min
Rest of fluid volume is =1000-729
=271ml
Drop factor is =20 gtts/min
So rate is =Diseired volume× drop factor/time in min
=271×20/165
=32.84 drops/min
=33 drops / min aproxmately
So the IV pump rate is 33 drops/ min if the fluid is to completed on 07pm..
