a)Pneumonia:-
It is defined as inflammation with exudative solidification of the lung parenchyma generally acute.
Clinical features:-
Sudden onset of rigors followed by fever ,chest pain ,purulent productive cough and haemoptysis
Treatment :-
General measure
- Check airway,breathing & circulation
- Correct hypoxia with oxygen inhalation
- Treatment of pleuritic pain with mild analgesics like paracetamol or codeine
Antimicrobial treatment:-
-Administer antibiotics as soon as possible once diagnosis is confirmed
- Erythromycin, Azithromycin,levofloxacin,gemifloxacin should be given as per condition and investigation of the case.
b)Bronchial Asthma:-
It is a chronic inflammatory disease of the airways characterised by an increased responsiveness of the tracheobronchial tree to a variety of stimuli.
Clinical Features:-
Paroxysms of dyspnoea,Cough and wheezing
Treatment and management:-
-Avoidance of allergens
-Desensitisation or immunotherapy is done by repeated subcutaneous injection of gradually increasing doses of extracts of allergens.
--Drug therapy:- Drugs like Beta adrenoreceptor agonists,Methylxanthines , corticosteroids etc. can be used as per the case.
- Inhaler is also recommended as per the severity of the case
2)Digestive system:-
a) Gastro-oesophageal reflux disease (GERD) :-
When gastric contents reflux into the lower oesophagus due to failure of mechanism which prevent reflux if gastric contents into the oesophagus.
Clinical feature:-
- Heart burn, acid eructation
- Dysphagia to solid due to structures
- Painful swallowing etc.
Treatment :-
General management
- Weight reduction and cessation of smoking
- Small volume frequent feeds
- Avoid alcohol,fatty foods
Medical treatment
- Liquid antacid
- Ranitidine 150 mg
Surgical treatment
- Surgical resection of structures
- Fundoplication if cause is sliding hiatus hernia
b) Peptic ulcer:-
It refers to an ulcer in the lower oesophagus ,stomach or duodenum,in jejunum after surgical anastomosis to stomach and ileum adjacent to Meckel's diverticulum
It happens in 10 percentage of all adult males
Clinical Features
-Epigastric pain and so sharp is the pain that patient will localuse the site with one finger
--Hunger Pain-Pain occurs on empty stomach and is relieved by food or antacids
-Heart burn.dyspepsia,Fullness of abdomen
3) Urinary system :-
a)Urinary tract infection (UTI):-
It is associated with multiplication of organisms in the urinary tract and is defined by presence of more than 10 to the power 5 organisms/ mL in midstream sample of urine.
Clinical features:-
- Fever with chills and rigors
- Frequent of micturumict
- Dysuria
- Urgency
- Haematuria
- Suprapubic pain
Treatment:-
- Drink plenty of water ( more than 2 L/ day)
- Urinary analgesics and antispasmodic for detrusor spasm
-Antibiotic should be given as the reports of Urine culture
-In female maintenance of perineal hygiene is of vital importance
b) Glomerulonephriti:-
It is the inflammation of glomerular in which there is an immunologically mediated injury to glomeruli.
Clinical features:-
- Haematuria
- Red blood cell casts
- Oliguria
- Oedema
- Proteinuria
- Uraemia
-Hypertension
Treatment :-
-Treatment of acute condition includes rest,salt restriction,diuretics and antihypertensives
- Antibiotics are given in some cases
- Dialysis is essential in severe oliguria.fluid overload and hyperkalemia
