First of all the nurse should explain the procedure to the patient and make her or him calm and comfortable by relieving anxiety to develop a therapeutic relationship and cooperation with the patient and then do the assessment .

In this case , the nurse should document that the patient can't maintain the position of arm and closes the eyes tightly which might be due to pain and fear .
