1. Consider the nursing standards of practice and codes of conduct applicable to your peer's proposed interventions to deliver culturally sensitive care. Identify and explain how the standards and codes support your peer(s)’ interventions to provide patient centered care.

2. State what modifications you can suggest to the planned nursing interventions which would enhance culturally sensitive care for this patient.

Peer’s post:

Scenario:
Joan, a 25-year-old, was brought via ambulance to the emergency department after collapsing on the street. She was diagnosed with shock and a ruptured colon; the surgical team was alerted to the potential surgery. The physician prepared to obtain informed consent and began discussing the surgery and the treatment with the patient. The RN was present at the bedside. The patient stated she did “not want any blood transfusion”. Based upon her beliefs as a Jehovah’s Witnesses, it is against her beliefs to receive blood. Joan is refusing to consent for surgery unless, the surgeon promises “no blood transfusions”. The RN believes the patient should accept the surgery and transfusion thinking “If it were me I would proceed with the surgery and treatments proposed by the surgeon.” The surgeon can be heard speaking to a fellow surgeon about how to go about changing the patient’s decision.

Address the following:

State and discuss at least two considerations (legal and/or ethical) occurring in this scenario.
An ethical consideration in our scenario is the nurse attitude towards the patient wishes of not accepting a blood transfusion during the surgery. The RN has the responsibility to support a patient’s right to refuse treatment, not to provide her personal view (Hinkle and Cheever, 2018). Another ethical consideration is the action of the surgeon verbalizing his intentions on finding a way to change the patient’s decision. Health care providers have the responsibility to honor a patient’s health decision and to try to provide other possible health alternative (Hinkle and Cheever, 2018).

How can the RN support the patient’s decision when the beliefs of the patient are contrary to their own?
The RN has the responsibility to provide health care in a holistic approach, this including spiritual care. Catalano (2015) explains that for an effective spiritual care, a healthcare provider needs to show no judgement and to provide necessary care that shows patient that he or she is significant (Catalano, 2015). The RN needs to demonstrate open-mindedness, sensitivity and tolerance with the patient’s healthcare wishes and communicate with the patient in a manner that provides hope and reassurance (Catalano, 2015).

List three (3) interventions, with supporting rationales, the RN would perform to ensure the delivery of culturally sensitive care.
1.Show respect for the patient’s religious values. This action demonstrates to the patient that you recognize their believes and still provide the best appropriate health care. This action helps build trust and can help with patient compliance (Catalano, 2015).

2.Behave in a manner that does not impose personal beliefs and values (Catalano, 2015). The RN should not response to patient’s cultural practices or treatment decisions with her personal opinion or opposing views.

3.Provide patient with time/resources and possible accommodation to support cultural needs (Treas et al., 2018). For example, providing patients with cultural/religious dietary needs, time for religious practice and native language TV channels whenever possible or available.

1. An ethical consideration in this case is the nurse attitude towards the patient decision of not accepting blood transfusion during the surgery. Nurse has the responsibility to support a patient’s view , not to provide her personal view.

Second ethical consideration is the surgeon's attempt of verbalizing his intentions on finding a way to change the patient’s decision. Health care providers have the responsibility to respect a patient’s decision and to try to provide other health options.

2. The RN has the responsibility to provide health care in a holistic approach, this including physical, mental, social and spiritual care. Catalano said that for an effective care, a healthcare provider needs to show no judgement and to provide necessary care that shows patient that he or she is significant. The nurse should be open-minded, sensitive and tolerance with the patient’s healthcare wishes and should communicate with the patient in a manner that provides hope and reassurance .

3. Three interventions are :-

1. Respect patient’s religious values. This action demonstrates to the patient that you recognize their believes and still provide the best appropriate health care. This action helps build trust and can help with patient compliance .

2.Behave in a manner that does not impose personal beliefs and values . No forcing . Nurse should not response to patient’s cultural practices or treatment decisions with her personal opinion or opposing views.

3.Provide patient with time/resources and possible accommodation to support cultural needs .For example, providing patients with time for religious practice and native language TV channels whenever possible etc .
