Case Study 7

A 45-year-old man went to see his doctor. He is a physical education teacher, who had done a “Tough Mudder” competition three days earlier. He had run 10 miles through mud, climbed several walls, one of which he fell down due to the fact he was slippery from all the mud, and taking an electrical volt to the head while in the water. He had felt sore for a couple of days, but you felt it was nothing unexpected considering the requirements of the race. He did have a nonproductive cough that he couldn’t seem to shake, but you figured was due to the running and exertion. What brought him into the office was that his muscles were now feeling even worse. His legs and buttocks were really sore. You suggest large doses of ibuprofen and a day or two off of school. The next day he calls back. He had become much worse overnight with a marked fever with shivering, excruciating headache, and eyes painful to move.

1.   Where did you go wrong? What is the most likely diagnosis?

2.   What was the most likely source of the infection? How was this infectious disease transmitted?

3.   How should you have diagnosed this disease?

4.   What are the therapeutic strategies that should be used?

5.   Do you think the Tough Mudder race contributed to his illness?
