OSHA requires the use of breathing apparatus when working in or around tanks containing traces of solvents. While demolishing an old tank, a contractor purchased several cylinders of compressed air, painted gray. After two days he found that he needed more cylinders and sent a truck for another cylinder. The driver returned with a black cylinder. None of the workers, including the man in charge of the breathing apparatus, noticed the change or, if they did, attached any importance to it. When the new cylinder was brought into use, a welder’s facepiece caught fire. Fortunately, he pulled it off at once and was not injured. What happened?
