A hospital readmission is a condition when a patient who was earlier discharged from a hospital is admitted again within a specified time interval. Readmission to hospital is generally considered as a measure of quality of hospital care. However, there are many factors which affect this situation such as seriousness of the underlying condition, severity, and predictability etc. It can prove as a vital criterion only if it is used carefully.

there are various methods for readmission assessment, but famous ones are The Centers for Medicare and Medicaid Services’ (CMS) all-cause readmission measure and the 3M Health Information System Division Potentially Preventable Readmissions (PPR) measure.

Hospitals can deal with higher readmission rates and deaths by enhanced communication, advanced care planning, medication safety. Further, by using home-based services, information technology, mental health care, caregiver support and community partnerships these situations can be kept under check.
