To success in case management, a nurse should requires a creative personality and good communication skills to case management.

1. RESPONSIBLE: Responsibility is one of the main aspect where the nurse should concentrate more and working in case management is a big responsibility. The nurse may be responsible for the medications of the client, therapies, rehabilitations, finanace management and involvement with the community and education.

2. CREATIVE PERSONALITY: As I said, creative personality helps the nurse to anticipate the unexpected, be creative in their work and remain resourceful.

3. TEAMWORK: Teamwork helps the nurse to provide best interventions to the patient. It creates a huge change in nurses involving in team work.

4. PATIENT-CENTRIC CARE: This helps the nurse to understand whatever information needed in disease management and core management of multiple conditions.

5. ENGAGING PATIENTS IN SELF-MANAGEMENT: Giving appropriate health education, self-monitoring of patients helps the nurse to engage in all preventive actions before admitted to the hospital.
