ALLOPURINOL

Pharmacologic action: Allopurinol inhibits xanthine oxidase, enzyme responsible for conversion of hypoxanthine to xanthine and of xanthine to uric acid.

Therapeutic use: It is used in primary or secondary gout in cancer patients receiving cancer therapy that causes elevations in serum and urinary uric acid levels.

Complications: hypersenstivity reactions, pericarditis,aplastic, hemolytic anemia ,hemorrhagic pancreatitis are some of the rare complications.

Medical administration: It is available in tablet and in injection form.

minium dose is 100- 200mg per day orally and maximum dose is 800mg per day orally.

Does need to be modified in renal failure patients.

Containdications/Precautions:

Contraindicated in patients who are hypersensitive to allopurinol. Discontinue the drug at the first sign of allergic reaction.

caution is needed in patients with renal impairment.

Maintain adequate intake of fluids.

Interactions:Severe bone marrow supression can occur when taken along with azothioprine as allopurinol potentiates the effects of thiopurines.

When taken along with ACE inhibitors , there is an increased risk of hypersensitivity reactions.

when taken along with warfarin, allopurinol inhibits the metabolism of warfarin thus prolonging its anticoagulating effect.

Nursing Interventions:

Nurses should encourage the patient to take adequate fluid intake.

Should monitor for joint pain and swelling.

Client education:

should educate the patient about acute gout flare ups that can occur in first few months of therapy.

should educate the patient regarding the common side effects like stomach upset,nausea,diarrhea, confusion etc.

Evaluation of medication effectiveness: Monitor the serum uric acid levels in regular intervals.
