pharmacological action of Cyclobenzaprine is a centrally acting muscle relaxant. Cyclobenzaprine is a 5HT2 receptor antagonist; it relieves muscle spasm through action on the central nervous system at the brain stem, rather than targeting the peripheral nervous system or muscles themselves.

Protein binding: 93%

The exact mechanism of action of cyclobenzaprine has not been fully determined, but this drug seems to primarily act at the brain stem to reduce tonic somatic motor activity, influencing both gamma and alpha motor neurons leading to a reduction in muscle spasms.

Therapeutic uses--

Cyclobenzaprine is a muscle relaxant. It works by blocking nerve impulses (or pain sensations) that are sent to your brain. Cyclobenzaprine is used together with rest and physical therapy to treat skeletal muscle conditions such as pain or injury.

Complications-

The most common effects associated with cyclobenzaprine overdose are drowsiness and tachycardia. Less frequent manifestations include tremor, agitation, coma, ataxia, hypertension, slurred speech, confusion, dizziness, nausea, vomiting, and hallucinations.

Administration-

Flexeril (Cyclobenzaprine Hcl)

For most patients, the recommended dose of FLEXERIL is 5 mg three times a day. Based on individual patient response, the dose may be increased to 10 mg three times a day. Use of FLEXERIL for periods longer than two or three weeks is not recommended.

Contraindications-

Some products that may interact with this drug include: tricyclic antidepressants (such as amitriptyline, imipramine).

Taking MAO inhibitors with this medication may cause a serious (possibly fatal) drug interaction. Avoid taking MAO inhibitors (isocarboxazid, linezolid, methylene blue, moclobemide, phenelzine, procarbazine, rasagiline, safinamide, selegiline, tranylcypromine) during treatment with this medication. Most MAO inhibitors should also not be taken for two weeks before treatment with this medication. Ask your doctor when to start or stop taking this medication.

Assessment

  • History: Hypersensitivity to cyclobenzaprine, acute recovery phase of MI, arrhythmias, CHF, hyperthyroidism, urinary retention, angle-closure glaucoma, increased IOP, lactation
  • Physical: Orientation, affect, ophthalmic examination (tonometry); bowel sounds, normal GI output; prostate palpation, normal voiding pattern; thyroid function tests

Interventions

  • Arrange for analgesics if headache occurs.

Teaching points

  • Take this drug exactly as prescribed. Do not take a higher dosage.
  • Avoid alcohol, sleep-inducing, or over-the-counter drugs; these may cause dangerous effects.
  • You may experience these side effects: Drowsiness, dizziness, blurred vision (avoid driving or engaging in activities that require alertness); dyspepsia (take drug with food; eat frequent small meals); dry mouth (suck sugarless lozenges or ice chips).
  • Report urinary retention or difficulty voiding, pale stools, yellow skin or eyes

Patient education--

Talk with your doctor before you drink alcohol or use other drugs and natural products that slow your actions. Do not take Flexeril (cyclobenzaprine tablets) for longer than you were told by your doctor. This medicine is used with rest, PT (physical therapy), pain drugs, and other therapies.

Cyclobenzaprine effectiveness

Cyclobenzaprine is eliminated quite slowly, with an effective half-life of 18 hours (range 8-37 hours; n=18); plasma clearance is 0.7 L/min. The plasma concentration of cyclobenzaprine is generally higher in the elderly and in patients with hepatic impairment.
