a 67y/o with type 2 diabetes, African American male, no allergies , family history of hypertension , CVA , Coronary Artery disease, healthy weight for the elderly male.

Just a Teaching plan is needed.

Follow the guidelines :-

1 -Assessment -Document initials and sex of patient. Identify patient needs to be taught. Gather information such as age, culture, family support, language spoken, ability, disability, problems or needs, etc.) (ALREADY DONE)

2- Planning / Nursing Diagnosis -State what you will teach. Identify one Nursing Diagnosis and Goal. Include any special teaching aids or techniques that would be helpful with teaching. See addendum regarding how to Teaching Concepts.

3 -Interventions -Describe specifically what you will teach the patient. Use terminology that the patient will understand

4 -Evaluation -What was the response to your teaching? Explain how you knew the patient did or did not learn what you were teaching?

ASSESSMENT NURSING DIAGNOSIS PLANNING IMPLEMENTATION EVALUATION

Patient information.

* African american male.

* Age 67 years.

* No allergies.

* Family history of hypertension,CVA,heavy weight,coronary artery disease.

Knowledge deficit related to lack of exposure to the procedure for measuring blood glucose level.

Goals or objectives.

* Make the patient aware of what is a glucometer

*Understand Articles needed other than glucometer for measure glucose level.

*Understand how to use it for measure blood glucose level.

Teaching aid

* Videos about measure blood glucose level with the help of a glucometer.

Teaching method or technique.

* Lecture

Definition of glucometer.

an electronic equipment used to measure blood glucose level.

Articles needed other than glucometer.

* Lancet.

* Cotton balls.

* Testing strips.

* Alcohol swab.

* Disposible gloves.

Procedure

*Wash hands and wear disposible gloves.

* Remove a test strip from strip bottle and close it immediately to protect from light exposure and discolouration.

* Turn on the glucometer monitor.

* Select appropriate puncture site(finger tips) and take a lancet.

* Massage side of the finger towards puncture site to increase blood flow and clean i the area with alcohol swab.

* Hold lancet perpenticular to skin and prick the site.

* Remove first drop of blood because it contain large amount of serum.

* Then gently touch the next drop of blood to test strip.

* Insert test strip into the glucometer *in some glucometers drop of blood need to be applied to test strip which is alreadty been inserted inthe monitor.

* Record the findings get from glucometer.

* Apply pressure to puncture site with a cotton ball.

* Discard lancet in sharp disposible bottle and remove gloves.

* Wash hands with soap and water.
