When the security guard arrives, the residential advisor asks all of the students in the area to return to their rooms. The security guard unlocks the door and enters the room. They find the client sitting on the floor in the corner of the room. He is dirty and the room is a mess. There is a strong smell of urine When the security guard speaks to the client, he quietly tells the guard to leave or he will be sorry. The client looks away and refuses to answer any of the guard's questions. The university nurse is summoned to the room. 1. 1)Some of the safety considerations to be done by the nurse is

• To avoid transmission of infection as the patient can be a source of infection during to micturation
• To remove any harmful materials if any around the patient
• To converse in a low tone

2)The possible reason is that the patient is experiencing a schizoid personality disorder .In this condition the individuals tend to be lonely al ways not having any relationship with the society or making friends,detached from others.

3)The data related to his childhood, his social interaction, coping strategies .This will help the nurse to rule out the causative factors and aiding in his treatment.

4)Appropriate intervention for this patient is psychotherapy in a health care set up.He has to be conveyed gently that they are they are there to help him. In these cases patients have emotional coldness unreactive to any strong reactions by others. Medication to make him sleep will help.
