There may be a time when your office requests that you correspond with a patient via the mail. One example is providing a patient with normal lab results. In this exercise your physician has requested that you send a lab letter to a patient informing them of normal results. INSTRUCTIONS: In this activity you work at Portland Community Clinic and your physician, Dr Jill Hays has requested you send a letter to Ms. Alice Colhoun regarding the results of her recent echocardiogram. You are to advise the patient that the results were normal, and that she should return for another cardiac evaluation in one year. Dr. Hays wants you to express to Ms. Calhoun the importance of continuing annual echocardiograms, as well as to report any distressing symptoms and follow-up in office as needed. Your letter can be worded any way you wish but it must contain (at the minimum) the above information and must be professional and respectful, as well as using direct simple language and clear communication style that would be easily understood by a geriatric patient. Dear Ms Alice ,

Iam writing on behalf of Dr Hays to inform you regarding your recent Echocardiogram results, which showed a normal size , structure and movement of your heart . Dr Hays advice to perform annual echocardiogram testing to rule out any cardiac abnormalities and to give better understanding regarding your condition.Also ,early diagnosis will help to provide appropriate treatment options . If you have any symptoms such as shortness of breath, unusual sound in your heart ( heart murmur) , or chest pain immediately report to the clinic as soon as possible and follow up to the office as required. Please note that , you should return for another cardiac evaluation in one year .
