Assessment means collecting data or information from the clients, in the form of subjectiva details or objective data.

* after collecting information we need to frame diagnosis as based on clients information and our observations.

* this diagnosis should be there parts statement, or two part statement.

* actual and risk diagnosis we can formulate.

* after formulating nursing diagnosis, next plan interventions in priority based, even diagnosis also priority based.

* then implement these nursing interventions as a nursing care plan on clients problems.

Yes, during assessment when we are collecting subjective data from the client we will ask patient scenario questions, because we need clear information from client ,what exactly they are suffering.

For example patient is suffering from pain means we can assess the client in the scenario like OPQRSTU

* 'O' means onset of pain, we need to ask the client when did it begin? And how long does it lasts?

* 'P' provoking and palliating : it means what begins it on?

And what makes it better and what makes it worse?
