ASSESMENT.

This step is mostly about collecting data and using critical thinking skills.In this the process nurse can know the problem of the patient by collecting the data from the family and also taking vital signs,weight,height and also about their medical history.

DIAGNOSIS.

In this the nurses can know about the health condition of teh patient using clinical judgement.

PLANNING.

In this step they plan the goals and outcome of the patients.Care plan helps direction of care and plans help in increasing the communication,documentation of the patient condition.

IMPLEMENTATION.

In this step the plan is implemented.Care,health maintanence, rehabilitation are seen and the outcomes are documented.

EVALUATION.

The care plan should be evaluated by seeing the outcome and effectiveness of the plan and it should be modified if needed so that it can better suit the care required for the patient.
