In Accordance with Advanced Trauma Life Support (ATLS) guidelines, in any case of trauma the initial management is always ABCDE. Hence in the given scenario,even in the presence of a large cut the steps remain the Same (ABCDE)

Shock is to be treated even if present only after assessing airway and breathing

ABCDE explained.

A = Airway management. Airways could be blocked by a clot or fallen tongue. Rx procedure examples include chin lift, jaw thrust, tracheostomy etc

B= Breathing. Assess for bilateral chest raise and breath sounds. Administer Oxygen if available

C= Circulation. Assess Vitals and Heart sounds. Check for signs of shock (cold clammy skin, rapid feeble pulse etc). Control external bleed by applying firm pressure over wound. DO NOT START CLEANING THE WOUND. Two iv lines (14G/16G). If shock is present,start fluids immediately and arrange for blood. Fluids upto one liter is acceptable and above 1.5 liters is never recommended in cases of hemorrhagic shock.

D= Disablitlty evaluation. By neurological examination, Glascow coma scale and pupillary reflexes

E= Exposing the patient. Undress the patient. Look for other injuries. Assess joints, bones, abdomen. Make a note of identification marks
