You are an RN and you are assigned to care for the following clients.

I. Charlene is an 87 year old Alzheimer disease patient.

Strengths

-Se has a good personality (sweet and polite).

-She doesn’t appear to be in pain (no physical pain).

Concerns

-She has memory loss (concern about her decision making capacity).

-She is very confused raising concern for her safety and most of the activities of daily living.

-She is not alert and oriented to person (only remembers her first name but couldn’t remember the last name), place or situation.

-She is degenerated, cognitive impaired that she could not recognize her son anymore.

2. Ashley is a 21 year old college student who has been on home ventilator since birth.

Strengths

-She seems happy, focused, determined and believed in herself.

-She has a great family support and people who love her.

-She has an amazing attitude towards life and has a good sense of humor.

Concerns

-Oxygen toxicity.

- Risk for infections

-Risk for lung damage and collapsed lung.

- Side effects of medications.

3. Intestinal/ Digestion Disease- Intestinal motility

Strengths

-Loving, independent and positive about life.

-Ability to manage her fluid balance chart (intake and output).

Concerns

-Risk of infection from blood drawing and Hickman line.

-She looks fragile, thin and weak.

-Risk for postural orthostatic.

- She is mixing all her medications together and not flushing which can cause tube blocking of the TPN.

4.Stephi- Cystic Fibrosis disorder patient.

Strengths

-She is independent, brave and strong.

-Ability to manage the PICC line effectively for infection.

Concerns

-Frequent infection as a result of reproduction of mucus.

-Longer hospital staying an increase the risk of infection.

-She is bothered and annoyed for lack of privacy.
