The topic is health communication regarding completion of the documentation of medical records by the physician .

# The goals and outcome objectives of the meeting are :-

- To improve the documentation practice

- To aleivate the poor documentation practices

- To follow the standards of the documentation

- To provide the healthcare professional a brief training on how to write the documentation.

# The present situation or reason for holding the meeting is the increase in number of incomplete medical records which is a leading cause of miscommunication , misconduct and malpractice in the healthcare settings.

The audience also had the same analysis, that there is an increase in number of malpractices and on survey it was found that incomplete documentation by the health care staffs was the cause behind this malpractice and that's why this meeting on proper documentation is held .

# The strategies with action for proper documentation are:-

- proper training of staffs about the documentation procedure.

- strict monitoring of the medical records by the senior staffs including ward in charges , superintendent and consultants

- Inbetween surveillance by outer administrative officers .

- Strictly punishing (including suspension and fine) the staffs those who don't follow the standards of writing documentation .

- All staff members must have completed the training and had the certificate of good documentation.

After the implementation of these strategies evaluation has to be done to find the results ,whether these strategies were successful or not and if not successful then what additional concepts have to be taken care of should be identified .
